WebMay 12, 2011 · On cheaper cast alum wheels the steel insert thing really helps prevent cracks from hard use. I have used inserts on redrilled (new bolt pattern) 8-10 lug wheels on 1 ton trucks; put 100k miles on them with good success. Good shop that does it here - and ships all over the US regularly, but you may find closer. OP.

Check websites like J&L industrial or Enco for 60* countersinks, buy a multiflute like the 82 or 90 degree you have. It also has a broken bearing on that wheel, and they said they may need to replace the spindle. They quoted me a little over $500. They say that my bearing cracked and the vibration caused my lug nuts to all vibrate loose.

Has been for the last 13 years. So while I doubt it was the lugs, and find it more believable that in reinstalling your old lugs, you probably tightened them down properly.
